CMS proposes 2027 Medicare Advantage, Part D updates; insurers warn of benefit pressure

The Centers for Medicare & Medicaid Services released its Calendar Year 2027 Advance Notice outlining proposed methodological changes for Medicare Advantage (Part C) and Medicare Part D payments. If finalized, CMS said the policies would result in a projected net average year-over-year payment increase of 0.09% in Medicare Advantage payments for 2027, or more than $700 million in additional payments to plans.

The proposal immediately became a major health-policy flashpoint because Medicare Advantage now covers tens of millions of Americans and is a centerpiece of private-sector delivery for seniors and many people with disabilities. For plans, even small changes in the rate baseline can cascade into premiums, benefit design, supplemental offerings and provider contracting decisions that affect what enrollees experience at the point of care.

CMS emphasized that the annual Advance Notice includes routine and technical updates intended to improve payment accuracy and sustainability, with the overall projection reflecting multiple components that influence plan payments. Those components include underlying cost growth, Star Ratings used to calculate quality bonus payments and updates to risk adjustment, among other policy levers.

Insurers and industry groups signaled that a smaller-than-expected increase could create pressure in 2027 benefit packages, particularly if medical cost trends remain elevated. Analysts have warned that plans may have to offset margin stress through tighter utilization management, narrower networks, reduced supplemental benefits or strategic exits from unprofitable areas.

The Advance Notice is the start of a formal process, not the final word. CMS can revise assumptions after receiving public comments and stakeholder feedback before final rates are set. Still, for consumers, employers and providers planning for 2027, the proposal is an early indicator that Medicare Advantage payment policy is entering a more constrained phase—with potential downstream impacts on access, plan choice and out-of-pocket costs depending on how the final rule is structured.
