A record-breaking surge in the Upstate
South Carolina’s measles outbreak has grown to 789 confirmed cases since September, with officials reporting nearly 600 new cases in just over a month. The outbreak is centered in northwestern Spartanburg County and is showing few signs of slowing, with schools and communities repeatedly affected by new exposure sites and quarantine waves.

The scale has surpassed the recorded total from Texas’ 2025 outbreak (762 cases), making it the largest US outbreak since measles was declared eliminated domestically more than two decades ago. Public health experts say the pattern fits measles’ ability to spread rapidly when it reaches pockets of unvaccinated or under-vaccinated people.

Measles is among the most contagious infectious diseases, and it can spread before a patient realizes they are infected. Complications can be severe, including pneumonia and brain swelling, and outbreaks strain school systems and healthcare capacity through contact tracing, isolation, and repeated quarantine events.
Officials and clinicians have warned that the United States’ measles elimination status is at risk if sustained transmission continues. Separate outbreaks have also been reported elsewhere, including along the Utah-Arizona border, reinforcing concerns that multi-state transmission chains could become harder to contain.
